TEAM Clinics Behavioral Medicine Visits

• Please make sure your child has their June, July, and August Behavioral Health appointments booked for their 30 day medication refill follow ups.

  Note:  If your nurse did not pre-book your child’s appointment, please notify us as directed below.

• You will receive a reminder text that will include the future date and time of your appointment. If you MUST reschedule, you will need to email medicalquestions@teamclinics.com or call 405-546-4130 (select option #4).

• If the parent/child cannot make the appointment, the appointment must be canceled or rescheduled 48hr prior to the appointment time, failure to do so will result in a “no show.”

• All no shows will be charged a no show fee.

• Refills will not be sent until the child is seen, with a parent/guardian present.

• On the day of your appointment, you will receive a text with your zoom link once your appointment has started. Click the link to enter the zoom appointment with your provider.

• Please make sure your child’s account has a current/updated cell phone number to receive the reminder notifications and zoom links.   https://app.teamclinics.com/login
